Le proglissement mandibulaire

RésuméLe proglissement mandibulaire est une anomalie fonctionnelle appartenant à la malocclusion de classe III selon la terminologie d’Angle. Ses origines sont multiples, allant de l’anomalie d’éruption des incisives de lait, ou définitives, à la dysfonction linguale au travers d’une langue basse. Malgré sa faible prévalence (3 à 7 %), elle doit être dépistée et traitée précocement (denture mixte ou temporaire) pour éviter qu’une anomalie fonctionnelle ne se transforme en anomalie squelettique. Il est important à ce stade de procéder à l’unique geste qui permet de poser le diagnostic différentiel : la manœuvre de De Névrezé ; elle permet d’obtenir une position de la mandibulaire plus rétruse minimisant les relations dentaires. En cas de prognathisme mandibulaire vrai, la manœuvre n’aboutit pas ; il n’y a aucune modification des rapports dentaires. Une phase thérapeutique interceptive permet de retrouver rapidement un guide antérieur correct et de réhabiliter la croissance des maxillaires.

The functional mandibular prognathism belong to the class III malocclusion according to the terminology of Angle. Its origins are multiple, from the abnormality of eruption of deciduous or definitive incisors to lingual dysfunction (low position of the tongue). In spite of its weak prevalence, it must be prematurely detected and treated (mixed or temporary teeth) to prevent a functional anomaly to become a skeletal anomaly. It is important at this stage to proceed to the unique gesture which allows making the differential diagnosis: it is the De Névrezé procedure; it allows obtaining a more retrusive position of the mandible to minimize the dental relations. In case of true mandibular prognathism, the maneuver does not succeed; there is no modification of the dental reports. An interceptive therapeutic phase allows finding quickly a previous correct guide and to rehabilitate the growth of jaws.
