Comment en est-on arrivé à suspendre l’obligation vaccinale du BCG ?

RésuméL’incidence de la tuberculose a fortement baissé depuis l’obligation de déclaration et la vaccination généralisée au milieu du xxe siècle. De nombreux pays ont suspendu le caractère obligatoire de la vaccination, préférant la réserver à des populations à risque. La France avait conservé la vaccination généralisée obligatoire, utilisant une injection intradermique dont la réalisation est difficile et les effets secondaires nombreux. Depuis 2004, différents avis sont favorables à une vaccination réservée aux populations à risque, en particulier celles originaires de pays à forte endémie de tuberculose. Ces avis préconisent également de renforcer le dépistage des sujets atteints de tuberculose. Madame Roselyne Bachelot, ministre de la Santé, a annoncé le 11 juillet 2007 la suspension du caractère obligatoire du BCG chez l’enfant et l’adolescent au profit d’une recommandation forte de vaccination des enfants les plus exposés à la tuberculose dès le 1er mois de vie. Parallèlement, un programme national de lutte contre la tuberculose a été lancé pour une période de 2 ans (2007–2009).
