Comment j’installe le patient pour une néphrolithotomie percutanée en position de lithotomie modifiée ?

RésuméIntroductionL’objectif de cet article est de décrire une nouvelle technique d’installation de patient en position de lithotomie modifiée pour néphrolithotomie percutanée.Matériels et méthodesLa technique est décrite en détail, en exposant les points clés de l’installation du patient et de l’organisation du bloc opératoire, à partir de l’expérience personnelle de l’auteur sur plus de 200 cas.RésultatsAu cours de la dernière décennie, la position de décubitus dorsal a été adoptée par un nombre croissant de centres urologiques pour la réalisation d’une chirurgie rénale percutanée. Cette installation a pour avantage d’éviter la nécessité de retourner le patient en position ventrale et aussi d’offrir simultanément un abord antérograde percutané et rétrograde transurétral aux voies urinaires. Néanmoins, les possibilités limitées pour l’apprentissage et l’absence de description précise ralentissent la diffusion de cette nouvelle technique.ConclusionsCette position a été adoptée non seulement par l’équipe chirurgicale mais aussi par l’équipe anesthésiste et par l’équipe paramédicale en raison de ses avantages (positionnement du patient, durée d’intervention, temps d’occupation du bloc opératoire…) qui définitivement ne nous feront pas revenir à une autre technique et qui vont permettre d’autres développements, entre autres, la mini-percutané.

This facility has the advantage of avoiding the need of returning the patient in prone position and also of providing simultaneous antegrade percutaneous and retrograde transurethral access to the urinary tract. However, the limited opportunities for learning and the lack of a precise description slow down the spread of this new technique.ConclusionsThis position has been adopted not only by the surgical team but also by the anesthesiologist team and paramedical staff due to its advantages (patient positioning, duration of intervention, operating room occupation time…) that definitely will not make us go back to another technique and which will allow further developments, especially mini-percutaneous nephrolithotomy.
