La préservation du sphincter strié dans la prostatectomie totale rétropubienne

RésuméPour que le risque d’incontinence urinaire après prostatectomie totale devienne nul ou négligeable, le traumatisme sphinctérien engendré par la section sphinctérienne doit être totalement réversible. Cette exigence technique impose de respecter tous les éléments anatomiques qui participent au maintien de la structure sphinctérienne et de sa fonction c’est-à-dire le plexus veineux dorsal, le noyau fibreux central du périnée et le fascia pelvien latéral. La technique opératoire se déroule en quatre étapes successives, où toute dissection susceptible de fragiliser l’unité anatomique sphinctérienne (UAS) est interdite. Les repères anatomiques sont les points déterminants à connaitre puisque aucune section ne peut être engagée si ces repères ne sont pas assurés. À la fin de ce temps opératoire, le manchon sphinctérien doit être circonscrit par un environnement anatomique intact, rendant obsolète toute idée de reconstruction sphinctérienne.

To avoid the risk of urinary incontinence following radical prostatectomy, the modifications caused by the division of the urethra must be completely reversible. The anatomical environment, which surrounds the urethra such as the dorsal venous complex, the lateral pelvic fascia and the central tendon of the perineum, forms a frame, which provides the strength of the striated sphincter and must be preserved. In order to avoid damaging the striated sphincter, the transection of the urethra is divided into four surgical steps and forbids the dissection of the surrounding anatomical structure. The section may only be performed if the anatomical markers are assured. At the end of the technique, the frame of the striated sphincter must remain intact avoiding any need for reconstruction.
