Curiethérapie du cancer de prostate : quel rôle pour l'IRM ?

Currently, MRI is seldom used in the management of patients who are candidates to prostate cancer brachytherapy. Multiparametric MRI could though improve patient selection by detecting macroscopic extraglandular invasion (fortunately rare in this population) and/or tumors whose size and aggressiveness have been underestimated by the classical work-up (digital rectal examination, PSA level and transrectal biopsy). MRI could also improve the evaluation of the dose actually delivered to the gland and the detection of local cancer recurrences. Finally, MRI could influence the treatment planning and allow the development of focal approaches either in first-line therapies or for salvage treatments of local recurrences after radiotherapy.
