RésuméLa tomographie par émission de positons (TEP) est extrêmement intéressante pour optimiser la balistique en radiothérapie pulmonaire et possiblement pour délivrer une dose hétérogène ou pour adapter la dose de radiothérapie en cours de traitement. Seule la place de la TEP-scanographie au fluorodésoxyglucose (FDG) est validée dans le bilan d'extension et la planification de la radiothérapie. Les méthodes de segmentation optimales restent à définir, tout comme l'intérêt du « dose painting » à partir des images pré-thérapeutiques du métabolisme ou de l'hypoxie et l'intérêt d'une replanification à partir des imageries per-thérapeutiques.
Functional imaging with positron emission tomography (PET) is interesting to optimize lung radiotherapy planning, and probably to deliver a heterogeneous dose or adapt the radiation dose during treatment. Only fluorodeoxyglucose (FDG) PET-computed tomography (CT) is validated for staging lung cancer and planning radiotherapy. The optimal segmentation methods remain to be defined as well as the interest of “dose painting” from pre-treatment PET (metabolism: FDG) or hypoxia (fluoromisonidazole: FMISO) and the interest of replanning based on pertherapeutic PET.
