The Care Plan is there to enlighten the multidisciplinary team who care for a severely disabled person. As far as severe disability (“polyhandicap”) is concerned, the complexity of the deficiencies as well as the health issues make this work a hard one. The Basal Stimulation approach, developed by Andreas Fröhlich, gives us models which allow us to determine the severely disabled adults or children's skills. This approach allows us as well to choose goals to help their development. The everyday life activities are, for the severely disabled person and the team who care for them, fundamental moments of the Care Plan application.
