Arthroscopy is no longer a “professor's toy” as it was in its early days. During the past decades, it had been established as a well-accepted minimal-invasive technique in the field of orthopedic as well as trauma surgery. However, arthroscopic fracture management of the hind foot, either as an entire arthroscopic approach or as an arthroscopic assist of open reduction, has gained more and more interest as recently as the past decade. Although the advantages of this technique like minimal-invasive surgical approach and improved visualization of the fracture zone are obvious, no compromises in fracture reduction or “internal” secondary collateral damage of the cartilage are acceptable. If arthroscopic fracture care is not feasible, a responsible conversion to an open approach should be undertaken instead of becoming obsessed for improving one's arthroscopic learning curve.
