CÉLULAS MADRE: FUNDAMENTOS Y REVISIÓN DE LA EXPERIENCIA CLÍNICA EN ENFERMEDADES HEPÁTICAS

RESUMENEn la actualidad, el trasplante hepático es el único tratamiento curativo para enfermedades como la cirrosis en etapas avanzadas y falla hepática fulminante. Sin embargo, la escasez de donantes, su alto costo y la morbimortalidad asociada son limitaciones importantes de este procedimiento. A causa de lo anterior surge la idea de utilizar células madre como terapia alternativa al trasplante hepático. Numerosos tipos de células madre exhiben potencial para desarrollarse en hepatocitos viables: células madre embrionarias, células progenitoras hepáticas, células madre derivadas de la médula ósea, entre otras. A su vez, existe una gran variedad de estudios experimentales y clínicos que evalúan el tratamiento con células madre en diferentes escenarios. Si bien muchos de estos estudios han demostrado mejoría en algunos de los parámetros de la función hepática a través de diferentes mecanismos, aún se desconocen los beneficios y efectos colaterales reales de esta terapia. Por otro lado, el corto periodo de seguimiento y la heterogeneidad de los estudios dificultan la comparación de estos y la obtención de conclusiones científicas. Estudios homogéneos, prospectivos, con un mayor número de pacientes y seguimiento prolongado permitirán definir el rol de esta terapia a futuro.

SUMMARYCurrently liver transplantation is the only curative treatment for diseases such as cirrhosis in advanced stages and acute liver failure. However, the shortage of donors, its high cost and associated morbidity and mortality are important limitations of this procedure. In order to avoid or reduce these limitations, therapy with stem cells could be used as an alternative to liver transplantation. Numerous types of stem cells exhibit potential to develop into viable hepatocytes: embryonic stem cells, liver progenitor cells, bone marrow stem cells, among others. There are a variety of experimental and clinical studies evaluating treatment with stem cells in different scenarios. Many of these studies have shown improvement in some parameters of liver function, however, the real benefits and side effects of this therapy are still unknown. On the other hand, the short follow-up and the heterogeneity of these studies do not allow to compare the results or to draw scientific conclusions. Homogeneous, prospective studies, with larger sample size and longer follow-up will allow defining the role of stem cell therapy in the future.
