ZusammenfassungWir führten eine prospektive Studie an Patienten mit einer operativ behandelten Hallux valgus-Fehlstellung durch, um den Effekt von zwei postoperativen Orthesen zu vergleichen.Die Daten von 69 Patienten wurden 4 und 12 Wochen postoperativ erhoben und beinhalteten klinische und radiologische Befunde (Hallux valgus-Winkel, Intermetatarsalwinkel I/II) sowie eine Schmerzbeurteilung (visuelle Analogskala) und die Patientenzufriedenheit bezüglich des Tragekomforts der jeweiligen Bandage. Die Bandage (“ERGO”) wurde individuell durch die Ergotherapeuten unseres Hauses angefertigt und besteht aus einer zirkulären Mittelfußmanschette sowie halbelastischen Textilzügeln für den Großzeh bzw. die Kleinzehen. Das Bandagensystem (“DARCO”) besitzt zusätzlich eine Metallschiene zum Erhalt der Stellungskorrektur des Großzehs.Im postoperativen Follow-up wiesen der Hallux valgus-Winkel, der Intermetatarsalwinkel I/II und die Schmerzintensität in beiden Gruppen eine signifikante Reduktion auf. Die klinischen und radiologischen Ergebnisse beider Gruppen zeigten keine Unterschiede. Auch die subjektive Patienteneinschätzung der Orthesen ließ keine Unterschiede erkennen.Aufgrund der guten Patientencompliance und hohen Akzeptanz empfehlen wir den routinemäßigen Gebrauch einer Redressionsbandage, um die postoperative Korrekturstellung des Großzehs zu erhalten. Dabei erscheint es unerheblich, ob es sich um eine reine Texilbandage oder eine semirigide Orthese handelt.

SummaryWe performed a prospective study on patients with hallux valgus surgery to compare the effect of two different orthoses applied routinely after the operation.Evaluation of 69 patients after a follow-up of 4 and 12 weeks included a clinical and radiological assessment (hallux valgus angle and first intermetatarsal angle), pain assessment (visual analog scale) and the patient's impression referring to the daily use of the orthosis. The orthosis type “ERGO” was manufactured by occupational therapists. It involves a metatarsal bandage and additional straps for the great toe and the lesser toes. The toe alignment splint “DARCO” was a customized soft bandage with a metal splint to attain a straight hallux position.During the postoperative follow-up period the hallux valgus angle, the first intermetatarsal angle, and the pain level revealed a significant decrease in both groups. Comparing the clinical and radiological results in both groups we found no differences. Even concerning the patient's subjective assessment the two orthosis revealed no significant differences.Attributed to the patient's compliance and the acceptance of the bandage we strongly recommend the routine use of a postoperative toe alignment orthosis to maintain a straight hallux position postoperatively. According to our study the type of the bandage (soft or semirigid) seems not to be crucial.
