► Children with absence epilepsy demonstrate impaired interictal attention. ► Impaired attention correlates with decreased BOLD fMRI in medial frontal cortex (MFC). ► Connectivity between right anterior insula and MFC is impaired in absence epilepsy. ► This provides a functional anatomical basis for impaired attention in absence epilepsy.
