La contribution infirmière à la prise en charge multidisciplinaire des patients vivant avec une néphropathie diabétique : une revue intégrative de la littérature

Diabetes and kidney disease are global health concerns. Diabetic nephropathy shows a constantly increasing prevalence. Despite advances in pharmacological and management strategies, diabetic kidney disease remains associated with high morbidity and mortality. To reduce associated cardiovascular complications and preserve their renal function, patients with diabetic nephropathy are expected to adopt complex treatment regimens and engage in their own self-care activities. Patients' non-adhesion to the treatment is thought to be the major cause for the poor control and the occurrence of complications. Multidisciplinary management, in which the nurse plays a central role becomes recommended for improving adherence. The nurse helps patients in developing their self-care abilities. She guides them in symptoms monitoring, in establishment of their priority care goals and in the development of problem-solving techniques. She coordinates patients' care, provides follow-up using reminders and monitoring of patients' progress towards the achievement of the set objectives through clinic or home visits.
