The term metatarsalgia describes several disorders of the foot, causing a plantar located pain. To get an accurate diagnosis, a profound theoretical knowledge as well as a precise clinical examination and a purposeful imaging is required, to initiate an adequate therapy. Clinically important in the development of a metatarsalgia is the bony and articular architecture of fore- and midfoot as well as knowledge on the load of the first ray and central metatarsals. Furthermore, the pathology of the plantar plate and the plantar fad pad plays an important role in the development of a metatarsalgia.
