Éducation thérapeutique et maillage territorial pour la prise en charge du diabète de l'enfant dans le nord-est de l'Île-de-France : l'équipe Di@bétomobile

Di@bétomobile is a mobile multiprofessional team working in cooperation with the pediatric units, in the North and East of the Île-de-France region, to complement the therapeutic education offered to the children with type 1 diabetes and those around them. The team proposes a therapeutic education in groups through various programs crafted from the analysis of the care teams, children and family needs. The learning methods and educational tools are adapted to the children's development; the needs are regularly reassessed from the diagnosis towards the transition to adult units.
