The natural evolution of valvular heart diseases has been studied for decades. Demographic changes, the patterns of presentation of valvular diseases and surgical progress do change steadily. In order to help in the decision-making process, guidelines for medical and surgical management of the heart valves diseases have been issued by expert committees. In this paper we will review the latest guidelines, new pathophysiological concepts and surgical indications.
