Étude de facteurs influençant les taux de NTproBNP et corrélation avec la fonction ventriculaire gauche

NTproBNP has proved its power as an exclusion marker in diagnosis of congestive heart failure. In this study, from patients admitted to the Hospital of Cardiology, several factors determining in NTproBNP concentrations (age, sex, renal function and cardiac necrosis) were evaluated with proBNP assay on Elecsys 20.10 (Roche Diagnostics, Meylan, France). NTproBNP levels increase of 695 ng/l (patients lower than 50 years), to 2002 (more than 80 years), of 622 ng/l (patients without renal dysfunction) to 6278 ng/l (severe renal insufficiency). These factors, as renal function, are crucial for NTproBNP results interpretation. From 39 echocardiography with LVEF measurements, at 250 ng/l, cut-off excluding a cardiac dysfunction and at 1200 ng/l with a high probability of heart failure, the negative predictive values of NTproBNP were respectively 100% and 85%. A negative correlation between LVEF and NTproBNP (R = -0.38) was observed. Then, NTproBNP can be used in the screening of patients with left ventricular dysfunction, but its interpretation in diagnostic of heart failure needs to include factors like age, sex and renal function.
