► More physically active people have lower risk of overt and, possibly, covert stroke. ► Damage after ischemia and perfusion is reduced in treadmill trained animals. ► The optimal exercise dose for ischemic stroke prevention is still unknown. ► Studies should examine whether there is acute protection after one exercise bout.
