► We conduct a comprehensive comparison of lateral structure of clinical surfactants. ► Clinical surfactants are compared to synthetic and bovine natural surfactants. ► All surfactant films form multilayer structures regardless of the DPPC content. ► Cholesterol regulates phospholipid phase separation and surfactant film collapse.
