La machine et l’hospitalité : un dilemme incessant tout au long des parcours-patients

RésuméDans un souci d’efficience des soins, les établissements de santé doivent (ré)organiser les flux de patients entrants et sortants au sein de parcours-patients. Car les établissements pour être efficaces doivent savoir parler de la maladie d’un point de vue épistémologique, mais aussi d’un point de vue réglé. Cette rationalisation des soins doit permettre de penser l’action avant ou après-coup, et non dans la surprise de l’agir, et éviter la survenue d’un événement indésirable (Éi). Un Éi qu’il faut anticiper, cartographier, déclarer dans des fiches incidents et analyser en Comité pluridisciplinaire afin d’éviter qu’il ne se reproduise. Ici apparaît notre principale inquiétude face au dispositif qualité et gestion des risques : que va-t-il rester de l’Événement au sein des machines à guérir hospitalières ? Un Événement en médecine défini comme « ce qui est et ne devrait pas être, mais dont nous ne pouvons pas dire pourquoi il est ». Mais dans notre société technicienne, l’Événement n’est plus vécu comme un événement fort du soin, mais comme un Éi, un grain de sable qu’il s’agit ensuite d’éradiquer. Un risque majeur surgit alors : que plus rien ne puisse arriver. Un avenir programmé, calculable par des ingénieurs des machines à guérir, ce serait l’annulation de l’avenir. Et pour que l’on ait foi dans cet avenir, il faut qu’il puisse rester incertain, irréductible à aucun programme ou machine pouvant mettre à mal l’accueil de l’autre, l’hôte et in fine l’hospitalité. Une hospitalité conditionnelle et une hospitalité inconditionnelle qui doivent rester indissociables, s’auto-prescrire, mais qu’à une seule condition : que l’on puisse penser et la machine et l’Événement comme deux concepts compatibles, indissociables.

SummaryIn a care efficiency concerns, health institutions must (re)organize the flow of incoming and outgoing patients in patients journeys. For institutions to be effective, they must know how to talk about the disease from an epistemological point of view, but also from a set point. This rationalization of care must allow to think the action before or after the fact, not in the surprise of the act and avoid the occurrence of an adverse event (AE). But in our technological society, the event is no longer experienced as a strong care event, but as an AE, a grain of sand that then needs to be eradicated. A major risk then arises: that nothing more can happen. A future planned, computable by curing machines’ engineers, it would be the cancellation of the future. And for us to have faith in the future, it must be able to remain uncertain, irreducible to any program or machine which can undermine the acceptance of the other, the host and in fine hospitality. A conditional hospitality and unconditional hospitality must remain inseparable, self-prescribe, but on one condition: that we can think and machine and Event compatible as two inseparable concepts.
