Anaesthetic community is especially sensitive to medications errors. The French anaesthetic society has edited guidelines to prevent them to occur. A method for evaluation of errors related to medications and medical devices (REMED) is available on www.adiph.org/sfpc. In the operating room, the percentage of medication errors leading to significant impairment is especially high compared to other settings (7.0% vs 2.4% according to MEDMARX from US Pharmacopeia). A review of medication and devices errors is comparable to a morbidity and mortality review but it is exclusively dedicated to this single topic and the consequences for patients. For health care providers, working in the operating theatre, a “good practice of pitfalls and errors” may guarantee prevention, diagnosis, interception, and recovery from medical errors, especially those related to medications.
