Medical files are structured to include reports and results of laboratory tests. Good quality of reporting in the medical file may impair the patient's management. Any ultrasound procedure refunded by the medical care insurance system needs to be recorded in the medical file. Not only ultrasound-guided regional anaesthesia reporting is not always documented, but there are great disparities in reporting. It is commonly believed that information provided by the anaesthesia sheet is enough to replace a dedicated report of the ultrasound-guided regional anaesthetic procedure. In this paper, we review legal regulations, medical reasons, and content of ultrasound-guided regional anaesthesia report.
