Myocardial infarction is defined by myocardial cells necrosis related to ischemia. The elevation of the plasma concentration of troponin is the biomarker of myocardial cells damage. Postoperative myocardial infarctions are more frequently related to myocardial ischemia due to increased oxygen consumption or decreased supply, but coronary artery thrombosis of plaque rupture may also be the cause. Most of postoperative myocardial infarction are asymptomatic and occur within the first 48 hours. Any increase in troponin plasma concentration must be considered as a risk factor of short, medium and long-term mortality. Ultrasensitive troponin I assay increases the number of postoperative patients with detected troponin plasma concentration leading to reconsider the definition of myocardial damage
