Perforations traumatiques de l’œsophage

RésuméRares mais graves, les perforations œsophagiennes traumatiques sont le plus souvent iatrogènes. Le principal pourvoyeur en est l’endoscopie digestive haute. Le diagnostic est suspecté devant l’apparition de signes cliniques évocateurs dans les suites d’un acte à risque. La tomodensitométrie cervico-thoraco-abdominale avec opacification digestive est l’examen clé dans cette indication. La stratégie thérapeutique, médicale, radiologique, endoscopique ou chirurgicale, dépend notamment des caractéristiques du patient et de la perforation et du délai diagnostique.
