Fire in the operating room is extremely rare but needs to be anticipated. Combustive is represented by inhaled gases such as oxygen and nitrous oxide. Disinfectant solutions are likely to be flammable especially when drying is not complete. Electrocautery is the heat source. In case of surgical field inflammation, it is mandatory to interrupt gases flow and to extract as soon as possible the inflamed bodies even if it is the tracheal tube. Fire extinguishers are not to be directed to the patient. Patients require management in burns units.
