Thrombopénie et grossesse

Thrombocytopenia complicates almost 10% of all pregnancies. In 75% of all cases, it reveals a gestational thrombocytopenia, which has no risk of haemorrhage and no consequence. However, thrombocytopenia can be associated with other causes with potentially severe complications: preeclampsia, hemolysis elevated liver enzyme and low platelets (HELLP) syndrome, immune thrombocytopenic purpura, thrombotic microangiopathy, disseminated intravascular coagulation or antiphospholipid syndrome. As the haemorrhagic risk is different according to each cause, an accurate etiologic diagnosis is important. Prescriptions of biological tests are not systematic and have to be adapted to each suspected etiology. Differentiation between immune thrombocytopenic purpura and gestational thrombocytopenia is mainly clinical. The anesthetic management is specific to each pathology.
