The use of endotracheal tubes in intensive care units is driven by goals different from those of anaesthesia. Endotracheal cuffs aim to prevent micro-inhalations susceptible to favour ventilation-associated pneumonia. Polyurethane low-pressure high-volume cylindrical cuffs and tapered cuffs have been designed to prevent ventilation-associated pneumonia. Internal cuff pressure must be monitored to stay between 20 and 30 cmH20. Subglottic continuous aspiration devices may reduce the incidence of ventilation-associated pneumonia to the expense of a high cost and tracheal lesions. Silver-coated endotracheal tubes are very expensive and their efficacy is not fully documented.
