Erreurs d'administration des médicaments : a-t-on progressé ?

Misses related to drug delivery are common and potentially costly. A monitoring committee has been dedicated to misses in drugs delivery by a French healthcare agency (ANSM). Most errors are related to wrong agent or wrong dose administration. Misleading conditioning is the leading cause (e.g. neostigmine/phenylephrine). Changes in ampoules labeling including notification of the dose and color encoding have resulted in a decrease reported incidence of mistakes. Improvement in conditioning needs to improve to decrease the incidence of errors up to “ready to use” delivery systems.
