Rekonstruktion fehlverheilter Chopart-Luxationsfrakturen

ZusammenfassungChopart-Luxationsfrakturen werden auch heute noch häufig initial übersehen oder fehlgedeutet und daher nicht adäquat behandelt. Fehlheilungen des Chopart-Gelenks führen aufgrund des Verlustes der essenziellen Gelenkfunktion und infolge der resultierenden dreidimensionalen Fehlstellungen zu erheblichen Einschränkungen der globalen Fußfunktion mit der raschen Entwicklung schmerzhafter posttraumatischer Arthrosen. Solange noch keine symptomatische Arthrose vorliegt, ist bei Gelenkfehlstellungen oder Pseudarthrosen, insbesondere des Talonavicular-Gelenks, die sekundäre anatomische Rekonstruktion und Osteosynthese indiziert. Pseudarthrosen und partielle Nekrosen des Os naviculare erfordern häufig eine zusätzliche Spongiosaplastik. In den meisten Fällen liegt eine Gelenkzerstörung mit manifester Arthrose vor, so dass eine Arthrodese des betroffenen Gelenks erforderlich wird, welche prinzipiell als reorientierende Arthrodese mit Korrektur der Achsen- und Längenverhältnisse ausgeführt wird. Für die Planung und Durchführung der Korrektur ist die Wiederherstellung der Balance zwischen medialer und lateraler Fußsäule von wesentlicher Bedeutung. Bei komplexen Fehlstellungen ist die korrigierende Chopart- bzw. Triple-Arthrodese indiziert.

If no symptomatic arthritis is present, joint displacement or non-unions may be subject to secondary anatomical reconstruction, especially at the talonavicular joint. Bone grafting frequently becomes necessary in cases of non-union or partial AVN of the navicular. In most cases joint distruction will have lead to manifest posttraumatic arthritis requiring fusion of the affected joint. Arthrodesis is always combined with axial realignment. For pre-operative planning and intra-operative control rebalancing of the medial and lateral columns of the foot is of utmost importance. In cases of complex malunions, corrective double or triple fusion is indicated.
