Arthroskopische Stabilisierung durch Thermoshrinking bei Sportlerschultern: mittelfristige Ergebnisse mit 2 – 8 Jahren Follow up

ZusammenfassungHintergrundDie arthroskopische Kapselschrumpfung ist ein seit über 15 Jahren etabliertes Operationsverfahren, welches in der Literatur in den letzten Jahren sehr kontrovers beurteilt wird. Neben Vorteilen des minimal-invasiven Vorgehens ohne Gewebedurchtrennung und eines einfachen operativen Handlings werden die Nachteile einer erhöhten Versagensrate und Komplikationen wie Nervenschädigungen und Chondrolyse diskutiert.Material und MethodeIn unserer retrospektiven Studie erfassten wir 96 Patienten mit einem Follow up von 2-8 Jahren. Wir stellten eine klare Indikationsstellung, führten ein standardisiertes operatives Vorgehen durch und hielten ein striktes Nachbehandlungsprotokoll ein.ErgebnisseDer korrelierte Constant Score betrug 90, der Rowe Score ebenfalls 90. Beim Rowe Score erreichten 90% ein exzellentes oder gutes, 4% ein befriedigendes und 6% ein schlechtes Ergebnis. Die Versagensrate lag bei 7,3%. Nervale Komplikationen oder Chondrolysen traten in unserem Kollektiv nicht auf.SchlussfolgerungZusammenfassend sehen wir die arthroskopische Kapselschrumpfung zur Behandlung der Schulterinstabilität als ein für spezielle Indikationen geeignetes und in der Hand des erfahrenen Schulterchirurgen erfolgreiches Verfahren.EvidenzebeneLevel III

BackgroundArthroscopic capsular thermal shrinkage has been a commonly used surgical procedure for more than 15 years, discussed controversely in literature. Benefits are the tissue-protecting minimally invasive procedure and the easy intraoperative handling. Drawbacks of the procedure are the supposedly higher failure rate and complications like nerve injury or chondrolysis.Materials and MethodsIn our study with 96 patients and a follow-up of 2 to 8 years we used differentiated indications, standard protocol for intra-operative procedure and post-operative therapy as well.ResultsThe correlated Constant-Score showed a mean of 90 as well as the Rowe-Score. 90% had excellent or good results, 4% were satisfactory and only 6% were considered failures. Our failure rate was 7,3%. We found no neural or chondral complications in our study.ConclusionsIn summary the arthroscopic capsular thermal shrinkage is an appropriate procedure in case of special indications to treat shoulder instability in the hand of experienced shoulder surgeons.Level of Evidence: Level III
