Die arthroskopische vordere Kreuzbandrekonstruktion – was ist gesichert 2009?

ZusammenfassungDie Rekonstruktion des vorderen Kreuzbandes gehört zu den häufigsten Operationen in der Sporttraumatologie und ist durch die Entwicklung der neuen Doppelbündeltechnik wieder aktuell in der Diskussion. Dennoch stellt derzeit die anatomische Einbündeltechnik noch den “State of The Art” in der Kreuzbandchirurgie dar, signifikante Verbesserungen konnten in der vorliegenden Literatur durch die Doppelbündeltechnik nicht gezeigt werden. Die Transplantatwahl und -fixation sowie die Indikation zur Operation stellen neben der korrekten anatomischen Transplantatplatzierung femoral und tibial die wichtigsten Säulen einer erfolgreichen Kreuzbandrekonstruktion dar.

SummaryACL reconstruction is one of the most frequent operations in sports medicine today. The development of the double-bundle technique raises new discussions about anatomic tunnel placement and ACL-reconstruction itself. Nevertheless, the anatomic single-bundle technique is still the “State of The Art” in ACL surgery. The literature is not able to document significantly better results for double bundle reconstructions. More importantly, factors such as the indication for surgery, the type of graft chosen and the graft fixation device define the outcome. The most important factor for a successful ACL-replacement continues to be the anatomic placement of the tunnels in femur and tibia, preferably in between the AM- and PL-bundles, in the single-bundle technique.
