Die Indikation zu Osteotomie, Schlittenprothese und Totalprothese am Kniegelenk

ZusammenfassungDie Kombination Achsenfehlstellung in der Frontalebene und einseitige Gonarthrose ist häufig und kann durch Osteotomie, Schlittenprothese oder Totalprothese behandelt werden. Wenn eine vorbestehende knöcherne Deformität zugrundeliegt, hat die achsenkorrigierende Osteotomie eine sehr gute Prognose. Liegt eine monokompartimentale Arthrose bei normaler Anatomie von Femur und Tibia vor, ist ein Achsenfehler durch intraartikulären Abschliff verursacht. Hier können durch einen unikondylären Gelenkersatz Achsenausrichtung und Bandspannung normalisiert werden. Kreuzbandinsuffizienzen können bei Osteotomien durch Änderungen der sagittalen Tibiakippung (slope) adressiert werden, wogegen Schlittenprothesen am instabilen Knie nicht eingesetzt werden sollten. Bei ausgeprägter Arthrose besteht hier die Indikation zur Totalprothese.
