There is increasing evidence that the coronary microcirculation (CM) plays an important role in the pathophysiology of numerous cardiac diseases. The CM function, along with the endothelial function, are the cornerstone for understanding some diseases like the cardiac syndrome X, as well as some of the complications of obstructive coronary artery disease, such as the «no reflow» phenomenon or post-revascularization angina. CM dysfunction is also the basis of the most important theories about other diseases like Tako-Tsubo syndrome, and also seems to contribute to the perpetuation and progression of both primary and secondary cardiomyopathies. Therefore, understanding it accurately will enable new early screening methods to be developed for cardiovascular diseases (CVD), addressing many CVD from the beginning, as well as the development of new therapies.
