Anticorps antiphospholipides et hémostase

RésuméLe syndrome des antiphospholipides est caractérisé par des manifestations cliniques dominées par des thromboses et des complications obstétricales récurrentes ainsi que par la présence persistante d’anticorps antiphospholipides. Les antiphospholipides regroupent une famille hétérogène d’anticorps qui présentent la particularité d’induire à la fois un état prothrombogène in vivo et un allongement paradoxal des tests de coagulation in vitro. Ces dernières années, nos connaissances des mécanismes physiopathologiques de ce syndrome ont connu une considérable avancée laissant espérer la mise au point de tests biologiques plus prédictifs du risque thrombotique. Cette revue décrit les différents mécanismes physiopathologiques par lesquels les antiphospholipides agissent sur l’hémostase conduisant ainsi à la survenue d’événements thrombotiques ainsi que leurs implications pour le diagnostic biologique et la thérapeutique.

The antiphospholipid syndrome is characterized by recurrent thrombosis and pregnancy morbidity in association with the persistent presence of autoantibodies called antiphospholipid antibodies. Antiphospholipids are a heterogeneous group of circulating autoantibodies associated with a risk of thrombosis and can paradoxically prolong in vitro the clotting times. In the past few years, significant improvement has been made in understanding the pathophysiologic mechanisms of the disease, thus raising the possibility of future predictive test for the risk of thrombosis. The aim of this review is to make an overview on how the antiphospholipids increase the risk of thrombosis and to provide an update of the laboratory diagnosis and the possible future therapies.
