Manifestations dermatologiques du syndrome des antiphospholipides

RésuméDe nombreuses manifestations dermatologiques ont été décrites au cours du syndrome des antiphospholipides (SAPL). La plus fréquente est le livédo ramifié aux mailles souvent fines, présent sur les membres et le tronc. Ce type de livédo est associé aux accidents vasculaires cérébraux et aux anomalies cardiaques valvulaires. Il appartient au phénotype artériel du SAPL. Les ulcérations circonscrites peuvent être la première manifestation du SAPL à type d’atrophie blanche. Ailleurs, elles sont secondaires à des thromboses veineuses itératives. Les nécroses extensives sont une des manifestations classiques du SAPL catastrophique. Les lésions à type de vascularite sont souvent trompeuses, rattachées au SAPL après les résultats de la biopsie cutanée. Au cours du lupus, l’anétodermie est toujours associée aux anticorps antiphospholipides.

A wide variety of dermatologic manifestations has been described in the antiphospholipid syndrome (APS). The most frequent skin lesion is livedo reticularis, present not only on the limbs but also on the trunk, with a fine irregular pattern. It belongs to the arterial subset of APS. Circumscribed ulcerations, resembling livedoid vasculitis, may be the first manifestation of APS. Ulcerations may also occur as a late complication of recurrent venous thrombosis. Extensive skin necrosis is a classic manifestation of catastrophic APS. Pseudo-vasculitis lesions are misdiagnosed if a skin biopsy is not performed, especially in the context of systemic lupus erythematosus. In systemic lupus erythematosus, primary anetoderma is always associated with antiphospholipid antibodies.
