Manifestations originales au cours de la polychondrite atrophiante : à propos de quatre cas

RésuméIntroductionLe diagnostic de polychondrite atrophiante (PCA) est difficile en raison de l’existence, en plus des chondrites caractéristiques, de manifestations extracartilagineuses.ObservationsÀ partir de l’analyse rétrospective des six dossiers de patients suivis au CHU de Nîmes pour polychondrite atrophiante entre 1995 et 2006, quatre ont été retenus en raison de l’existence de manifestations originales. Le premier associait des manifestations thromboemboliques au moment du diagnostic, puis une arthrite temporomandibulaire associée à une uvéite antérieure et une surdité bilatérale. Le deuxième, une kératite ponctuée avec surdité mixte, puis une vascularite leucocytoclasique. Le troisième, des manifestations thromboemboliques répétées avec un syndrome de Sweet et le quatrième, une présentation initiale avec une érythrodermie fébrile.ConclusionLes observations que nous présentons illustrent la difficulté du diagnostic de polychondrite atrophiante et la variété des atteintes extrachondritiques associées.

IntroductionThe diagnosis of relapsing polychondritis is difficult as various manifestations may be encountered aside the characteristic episodes of recurrent chondritis.Case reportsFrom the retrospective analysis of the medical charts of patients presenting with relapsing polychondritis seen at Nîmes hospital between 1995 to 2006, four were selected for their original extracartilaginous manifestations. Case 1: relapsing polychondritis was diagnosed at the time of a thromboembolic event associated with a right uveitis, left temporomandibular arthritis and bilateral sensorineural deafness. Case 2: relapsing polychondritis occurred in a patient with history of bilateral sensorineural deafness with punctuated keratitis followed by a relapsing cutaneous leucocytoclastic vasculitis. Case 3: relapsing polychondritis associated with recurrent thromboembolic disease and a Sweet's syndrome and case 4: relapsing polychondritis presenting with a febrile erythema.ConclusionThe reported observations highlight the difficulty of the initial diagnosis of relapsing chondritis and the variety of the extrachondritis manifestations that could be observed in this disease.
