SEMICYUC 2012. Recomendaciones para el manejo en cuidados intensivos de la pancreatitis aguda

ResumenObjetivoSe han producido grandes cambios en el manejo de la pancreatitis aguda desde la Conferencia de Consenso de Pamplona 2004. El objetivo ha sido revisar dichas recomendaciones y actualizarlas, para uniformizar el manejo integral de la pancreatitis aguda potencialmente grave en la UCI.ParticipantesMédicos intensivistas nacionales e internacionales, radiólogos, cirujanos, gastroenterólogos, médicos de los servicios de urgencias y otros médicos implicados en el tratamiento de la pancreatitis aguda.Niveles de evidencia y grados de recomendaciónSe ha seguido el método GRADE para su elaboración.Elaboración de las recomendacionesLa elección de jurados y vocales se realizó mediante convocatoria pública. Se ha realizado la revisión bibliográfica desde 2004 hasta la actualidad, y se han elaborado 16 bloques de preguntas sobre el manejo de la pancreatitis aguda en la UCI. Primeramente se han trabajado las preguntas por grupos, para posteriormente elaborar un documento único. Dicho documento se ha discutido y consensuado por vía informática, en el Congreso de la SEMICYUC, y finalmente en la Conferencia de Consenso realizada expresamente para su elaboración.ConclusionesSe han descrito 82 recomendaciones para el manejo de la pancreatitis aguda en la UCI. De ellas destacaríamos la nueva clasificación de la pancreatitis aguda basada en determinantes evolutivos, las nuevas técnicas quirúrgicas y las recomendaciones nutricionales.Nota. ObjectiveSignificant changes in the management of acute pancreatitis have taken place since the 2004 Pamplona Consensus Conference. The objective of this conference has been the revision and updating of the Conference recommendations, in order to unify the integral management of potentially severe acute pancreatitis in an ICU.ParticipantsSpanish and international intensive medicine physicians, radiologists, surgeons, gastroenterologists, emergency care physicians and other physicians involved in the treatment of acute pancreatitis.Levels of evidence and grades of recommendationThe GRADE method has been used for drawing them up.Drawing up the recommendationsThe selection of the committee members was performed by means of a public announcement. The bibliography has been revised from 2004 to the present day and 16 blocks of questions on acute pancreatitis in a ICU have been drawn up. Firstly, all the questions according to groups have been drawn up in order to prepare one document. This document has been debated and agreed upon by computer at the SEMICYUC Congress and lastly at the Consensus Conference which was held with the sole objective of drawing up these recommendations.ConclusionsEighty two recommendations for acute pancreatitis management in an ICU have been presented. Of these 84 recommendations, we would emphasize the new determinants-based classification of acute pancreatitis severity, new surgical techniques and nutritional recommendations.Note.
