Hémochromatose génétique : le rhumatologue en première ligne

RésuméL’hémochromatose génétique est dans la majorité des cas principalement due à la mutation p.Cys282Tyr du gène HFE, mais d’autres gènes peuvent être impliqués. Les progrès dans la connaissance du métabolisme du fer ont permis de mettre en évidence le rôle central de l’hepcidine. Les manifestations ostéoarticulaires de la maladie sont au premier plan tant au moment du diagnostic qu’à la phase d’état par leur retentissement sur la qualité de vie. Il s’agit, d’une part, du rhumatisme hémochromatosique qui constitue une des principales plaintes des patients et, d’autre part, de l’ostéoporose qui augmente le risque fracturaire. Les complications de la maladie engageant le pronostic vital de la maladie (atteinte hépatique, cardiaque, diabète) sont prévenues par la mise en place de saignées pour obtenir la désaturation. Ce traitement n’a généralement pas d’impact sur les manifestations articulaires de l’hémochromatose qui doivent cependant être prises en charge (traitements symptomatiques, traitements locaux). Il est important d’informer le patient sur la maladie et son retentissement pour lui et ses apparentés. Enfin, toute ostéo-arthropathie inexpliquée doit conduire à un dosage de la saturation de la transferrine et de la ferritine, permettant de diagnostiquer la surcharge en fer.

Hereditary hemochromatosis is mainly due to the mutation p.Cys282Tyr of the HFE gene, but other genes may be involved. Advances in the knowledge of iron metabolism have helped highlight the central role of hepcidin. Arthropathies and osteoporosis are at the forefront both at the time of diagnosis than to the phase state because of their impact on quality of life. Joint symptoms are one of the main complaints of patients; osteoporosis increases the risk of fracture. The life-threatening complications of the disease (liver, heart, diabetes) are preventable by the introduction of phlebotomies to obtain the desaturation. This treatment usually does not affect articular manifestations of hemochromatosis but these must be treated (symptomatic and local treatments). It is important to inform the patient about the disease and its repercussions for him and his relatives. Finally, any unexplained osteoarthropathy should lead to a determination of transferrin saturation and ferritin to diagnose iron overload.
