Lymphome rénal révélé par une insuffisance rénale aiguë

RésuméNous présentons deux observations de lymphome rénal révélé par une insuffisance rénale aiguë (IRA) qui reste une entité clinique rare. Dans l’observation 1, chez un jeune homme de 29 ans ayant une IRA, le diagnostic retenu était un lymphome rénal primitif d’immunophénotype B. Le patient est décédé après trois cures de chimiothérapie à cause d’une dissémination lymphomateuse rapide. Dans le second cas, il s’agit d’une infiltration lymphomateuse rénale de haut grade et d’immunophénotype B, chez une jeune femme de 25 ans, donnant une image scannographique inhabituelle de deux gros reins comprimant l’estomac. Le décès est survenu précocement avant l’instauration de toute thérapeutique. Dans les deux cas, le diagnostic a été affirmé par l’analyse anatomopathologique d’une biopsie rénale. La précocité du diagnostic est un élément clé du succès thérapeutique, toutefois la dissémination lymphomateuse est souvent rapide mettant en jeu le pronostic rénal et vital.

We present two cases of renal lymphoma revealed by acute renal failure (ARF), which remains a rare clinical entity. Case 1 was a 29-year-old man with an ARF. The diagnosis was a primitive kidney immunophenotype B lymphoma. The patient died after three courses of chemotherapy due to rapid spread lymphoma. The second case was a high-grade renal lymphomatous infiltration, with an unusual computer tomography image of two large kidneys compressing the stomach. Death happened early before initiating therapy. In both cases the diagnosis has been established by renal pathology. Early diagnosis is a key component of therapeutic success, however, the rapid spread of lymphoma worsened renal and vital prognosis.
