Syndrome hépatorénal

RésuméLe syndrome hépatorénal (SHR) est une insuffisance rénale aiguë fonctionnelle par vasoconstriction rénale qui survient chez le cirrhotique avec dysfonction vasculaire. La vasoconstriction rénale est réactionnelle à la vasodilatation artériolaire diffuse. Il existe deux types de SHR qui se distinguent selon l’évolution et le stade de l’insuffisance rénale et dont le pronostic est différent. La transplantation hépatique demeure le traitement de référence. Les traitements médicamenteux d’attente sont essentiellement représentés par les analogues de la vasopressine. La place de l’épuration extrarénale et des techniques de shunt intrahépatique portosystémique restent à préciser. Le pronostic du SHR est défavorable avec une mortalité à court terme élevée en l’absence de traitement.

SummaryThe hepatorenal syndrome (HRS) is an acute functional renal failure due to renal arterial vasoconstriction occurring in cirrhotic patients with vascular dysfunction. The renal arterial vasoconstriction is the result of diffuse arteriole vasodilatation.
