Influence du type d’orthèse de genou sur l’évolution clinique postopératoire d’une chirurgie du ligament croisé antérieur chez le sportif

RésuméObjectifsDéterminer l’influence du type d’orthèse sur l’évolution clinique postopératoire après chirurgie du ligament croisé antérieur (LCA) chez le sportif.Patients et méthodeLes auteurs ont analysé de façon prospective l’évolution de trois populations de ligamentoplasties : l’une ayant bénéficié d’une orthèse articulée, l’autre d’une attelle rigide et la dernière d’aucun moyen d’immobilisation. Les trois groupes ont suivi le même programme de rééducation pendant un mois en postopératoire. Les paramètres cliniques d’évaluation ont été la douleur, le périmètre rotulien, l’épanchement articulaire, les amplitudes articulaires, l’amyotrophie, le verrouillage du quadriceps et la qualité de la marche. Ont également été réalisées des évaluations par les scores PPLP et l’IKDC subjectif.RésultatsIl n’existait pas de différence significative à environ un mois de la chirurgie entre les trois groupes (407 patients), ni sur les paramètres cliniques, ni sur le score PPLP. Aucune rupture itérative n’a été retrouvée en postopératoire quel que soit le groupe. En revanche, il existait une différence significative concernant l’IKDC subjectif (p = 0,03) entre le groupe « attelle articulée » et « attelle rigide ».ConclusionAvec le même programme de rééducation active, l’évolution clinique a été identique avec ou sans attelle. La seule différence a été subjective (IKDC subjectif).

ObjectivesTo establish the influence of the type of brace on the postoperative clinical evolution after anterior cruciate ligament (ACL) surgery in competitive sportspeople.MethodsThe authors prospectively followed the evolution of three populations of ACL reconstruction: one who received a functional brace, the other a rigid brace and last no brace. The three groups followed the same rehabilitation program. The clinical parameters of evaluation were pain, perimeter patellar joint, range of motion, muscular atrophy, locking the quadriceps, and the quality of walking. We also used the IKDC subjective Knee Evaluation Form and PPLP scoring scale.ResultsThere is no clinically significant difference at about a month of surgery between the three groups (407 patients). There is no difference with the PPLP scoring scale. No repeat rupture is occurred postoperatively. There is a significant difference in the IKDC subjective (P = 0.03) between the group with functional brace and rigid brace.ConclusionWith the same active rehabilitation program, the clinical evolution is identical with or without brace. The only difference is subjective (IKDC subjective Knee Evaluation Form).
