Adénocarcinome papillaire sur tissu thyroïdien ectopique

RésuméObjectifsRapporter un cas d’adénocarcinome papillaire survenu sur tissu thyroïdien ectopique siégeant au niveau de l’os hyoïde avec une revue de la littérature.Matériel et méthodesUn patient de 57 ans consultait devant une masse cervicale antérieure, évoluant depuis environ un an, sans signes de compression locorégionale, ni signes de dysthyroïdie.RésultatsÀ l’examen, la tuméfaction était de consistance ferme, faisant corps avec l’os hyoïde, mobile par rapport aux différents plans, mais de limites imprécises, mesurant environ 8 cm de diamètre dans son grand axe. La glande thyroïde eutopique était en place, en position prétrachéale. La tomodensitométrie cervicale a objectivé un processus lésionnel ostéolytique centré sur l’os hyoïde avec une zone charnue de densité tissulaire et des calcifications. L’IRM cervicale a permis de réaliser un bilan d’extension plus précis de la masse en confirmant que cette dernière respecte les structures de voisinage, notamment l’endolarynx. Nous avons réalisé chez ce patient un abord cervical de la tumeur qui a permis une exérèse totale. L’étude anatomopathologique de la pièce opératoire était en faveur d’un adénocarcinome papillaire sur tissu thyroïdien ectopique. Le traitement a été complété par une thyroïdectomie totale dont l’étude histologique était normale.ConclusionsLa survenue d’un adénocarcinome papillaire sur un tissu thyroïdien ectopique est une situation très rare. Son diagnostic est avant tout histologique. Son traitement est essentiellement chirurgical, associé éventuellement à une irathérapie.

ObjectivesTo report a case of papillary adenocarcinoma occurring on ectopic thyroid tissue in the hyoid bone region.Material and methodsA 57-year-old patient consulted for a cervical mass appearing 1 year before, with no signs of compression. The thyroid function was normal.ResultsThe examination showed a firm tumefaction, fixed on the hyoid bone, with undetermined limits, measuring approximately 8 cm. The thyroid gland was in the normal pretracheal position. The cervical tomodensitometry objectified an osteolytic process centered on the hyoid bone with a fleshy zone of tissue density and calcifications. Cervical MRI provided a more precise assessment of the extension of the mass and confirmed the integrity of the other structures, in particular the endolarynx. The tumor was totally resected via a cervical approach. The pathologic study suggested a papillary adenocarcinoma on ectopic thyroid tissue. The treatment was completed by a total thyroidectomy with normal histological results.ConclusionsPapillary adenocarcinoma on ectopic thyroid is a very rare situation. Its diagnosis is histological. Its treatment is primarily surgical, sometimes associated with radioiodine I-131 therapy.
