RésuméLes mécanismes qui peuvent expliquer la consommation d’antibiotiques en pratique ambulatoire dépassent largement la microbiologie et l’épidémiologie. Des facteurs socioculturels (propension à consulter) et structurels (système libéral à l’acte), similaires en France et en Belgique, expliquent comment une offre de soins et une demande inappropriées s’auto-entretiennent pour une partie importante des infections respiratoires, plaçant ainsi les deux pays en haut du classement européen. Outre ces déterminants collectifs de prescription d’antibiotiques, la responsabilité individuelle des médecins généralistes est également discutée.
The mechanisms that might explain the antibiotic use in ambulatory practices go widely beyond microbiology and epidemiology. Sociocultural factors (help-seeking behaviour) and structural factors (liberal fee-for system), similar in France and Belgium, explain how an inappropriate health demand and an inappropriate supply are supporting each other, to keep the two countries high in European ranking. Beyond these collective factors of antibiotic use, the general practitioners’ individual responsibility is also discussed.
