La constipation de l’enfant : le point de vue du chirurgien pédiatre

RésuméLa constipation est un motif très fréquent de consultation en pédiatrie. Elle est définie comme l’émission de selles trop rares ; la notion de selles difficilement émises, car volumineuses ou très dures est également prise en compte. L’avis du chirurgien pédiatre est sollicité, avant tout, pour être certain qu’il n’y a pas d’anomalie organique relevant de la chirurgie, mais aussi lorsqu’une constipation « fonctionnelle » persiste malgré un traitement médical symptomatique. Le diagnostic de constipation fonctionnelle ne doit être retenu qu’après avoir éliminé une constipation secondaire, en particulier une anomalie chirurgicale. La manométrie anorectale est indiquée lorsque le traitement habituel de la constipation est inefficace ; elle constitue le premier temps d’une éventuelle rééducation par biofeedback. Le traitement de cette pathologie fonctionnelle est, avant tout, médical, la chirurgie ne s’adressant — secondairement — qu’à des cas très sélectionnés.

It is defined as the infrequent defecation, the notion of difficulty during defecation, because voluminous or abnormal stool consistency, is also taken into account. The anorectal manometry is indicated when the usual treatment of the constipation is inefficient; it constitutes the first time of a possible rehabilitation by biofeedback. The treatment of this functional pathology is before all medical, the surgery addressing — secondarily — of the selected cases.
