RésuméLe contrôle ciblé de la température (CCT) améliore le pronostic neurologique et de la survie des patients comateux admis à l'hôpital après un arrêt cardiaque. Il est recommandé de maintenir une température centrale, ciblée entre 32 et 36 °C, pendant au moins 24 h. Pour l'induction de l'hypothermie, la perfusion de volume important de soluté froid n'est pas recommandée. Le maintien autour de la température cible doit être précis et constant et le réchauffement doit être lent et contrôlé (maximum 0,5 °C/h). Le rebond hyperthermique lors de l'arrêt du CCT est fréquent, probablement délétère sur le plan neurologique et devra donc être contrôlé.
SummaryTargeted temperature management (TTM) has been shown to improve neurological outcomes and survival for patient still comatose after cardiac arrest. It is recommended selecting and maintaining a central temperature between 32 and 36 °C during TTM for at least 24 h. In the prehospital setting, cooling induction with rapid infusion of cold intravenous fluid is not recommended. Temperature should be maintained close to the target and the rewarming phase needs to be slow and controlled (maximum 0.5 °C/h). Rebound hyperthermia is common, probably associated with worse outcome, and as such, should be avoided.
