RésuméProposLe diagnostic différentiel entre la maladie tuberculose (granulomatose la plus fréquente à l'échelle mondiale) et la sarcoïdose (granulomatose rare) est important, car les traitements diffèrent complètement.MéthodesNous avons évalué prospectivement la présence de granulomes épithélioïdes à la biopsie de glandes salivaires accessoires (BGSA), critère d'orientation habituel pour le diagnostic de sarcoïdose, chez des patients atteints de tuberculose prouvée et vus consécutivement.RésultatsSur notre échantillon de 65 patients, 10,8 % des patients tuberculeux avaient des granulomes épithélioïdes sans nécrose caséeuse sur leur BGSA, indépendamment de la localisation de la tuberculose, du statut VIH du patient ou du caractère bacillifère de l'infection.ConclusionLa présence de granulomes épithélioïdes à la BGSA ne permet donc pas d'éliminer une maladie tuberculose lorsque le clinicien hésite entre cette affection et une sarcoïdose.
PurposeThe distinction between tuberculosis (TB), a worldwide infectious granulomatosis requiring specific antibiotic therapy, and sarcoidosis, a rare granulomatous disease that may require corticosteroids is not straightforward and may result in diagnostic and therapeutic delay.MethodsWe prospectively and consecutively evaluated the presence of epithelioid granulomas in minor salivary gland biopsy of 65Â consecutive patients with TB.ResultsIn our study, 10.8Â % of our TB patients had epithelioid granulomas without caseous necrosis identified in their minor salivary gland biopsy, regardless of the location of TB, HIV status and whether or not the sputum examination was positive for tuberculous bacilli.ConclusionThe presence of epithelioid granulomas in minor salivary gland biopsy may not be helpful to the clinician to rule out TB in a patient with suspected sarcoidosis.
