RésuméIntroductionL'hématopoïèse extra-médullaire est une tumeur bénigne rare, dont la localisation périrénale est exceptionnelle. Elle est secondaire à une stimulation médullaire, le plus souvent une hémolyse chronique chez l'enfant et l'adulte jeune, et un syndrome myéloprolifératif chez l'adulte d'âge mur.ObservationUn homme de 62 ans présentait une volumineuse tumeur périrénale bilatérale isolée. Les caractéristiques radiologiques et scintigraphiques (nanocolloïdes marqués au technétium et au chlorure d'indium) étaient en faveur d'un foyer d'hématopoïèse extra-médullaire. L'histologie était en faveur d'un myélolipome. La cause était une sphérocytose héréditaire atypique.ConclusionNotre observation est la première à décrire une forme frontière entre hématopoïèse extra-médullaire et myélolipome de localisation périrénale révélant une sphérocytose héréditaire atypique à l'âge adulte.
IntroductionMyelolipomas and extramedullary hematopoietic tumors are uncommon benign tumors. They are variably composed of mature adipose tissue and hematopoietic tissue. Myelolipoma is usually observed in the adrenal gland and extramedullary hematopoietic tumors in the liver and spleen but may occasionally be found within solid tumors.Case reportA 62-year-old man without previous haematological history presented with a voluminous solitary bilateral renal tumor. Contrast-enhanced ultrasound CT-scan and scintigraphy with technetium-99m-nanocolloid and indium-111-chloride bone marrow were highly suggestive of extramedullary hematopoietic tumor. CT-guided biopsy suggested a diagnosis of myelolipoma. An atypical hereditary spherocytosis, undiagnosed until now, was demonstrated.ConclusionWe report, for the first time to our knowledge, a border form between extramedullary hematopoiesis tumor and myelolipoma of renal localisation revealing a hereditary spherocytosis in an adult patient.
