Présentation atypique d'un déficit immun commun variable. À propos d'un cas

RésuméIntroductionL'association granulomatose - déficit immunitaire commun variable (DICV) - est bien connue des internistes. Mais l'association avec une leucémie à grands lymphocytes à grains (LGL) au cours du DICV est moins connue.ObservationNous rapportons l'observation d'une patiente de 50 ans suivie pour un DICV associée à une granulomatose. Au cours de son suivi, elle développait, d'une part, une atteinte pulmonaire non infectieuse de type « granulomatous lymphocytic interstitiel lung disease » (GLILD) et, d'autre part, une hyperlymphocytose chronique monoclonale faisant découvrir une leucémie à LGL.ConclusionIl s'agit, à notre connaissance, du premier cas rapporté d'association entre un DICV et une leucémie à LGL.

IntroductionThe association granulomatosis - combined variable immunodeficiency (CVID) - is well known from the clinicians. However, the association with a large granular lymphocyte (LGL) leukemia has not been yet reported.Case reportWe report a 50-year-old woman, followed for CVID associated with a granulomatous disease. During the follow-up, the patient developed a granulomatous lymphocytic interstitiel lung disease (GLILD). Secondarily, she presented a LGL leukemia.ConclusionTo our knowledge, this is the first reported case of an association between CVID and LGL leukemia.
