Shock séptico secundario a contusión pulmonar: ¿inmunomodulación sería la estrategia? Reporte de caso y revisión de la literatura

Pulmonary contusion is a recurrent finding after high energy traumas. Its treatment traditionally consists of support assistance, and is usually associated with an increase in mortality. A case is presented of a patient with a severe pulmonary contusion with haemodynamic impairment and pulmonary septic shock. It was refractory to antibiotic treatment, but showed a significant clinical improvement after immunomodulation therapy with enriched immunoglobulins, complimentary to the baseline therapy. The immunoglobulins therapy has been proposed as complementary in sepsis, however, the sepsis survival guideline does not recommend it as adjuvant therapy in sepsis. Recent accumulated evidence opens new perspectives for immunomodulatory therapy in the context of patients with septic shock.
