Proximal humeral fractures in children and teenagers are usually treated non-operatively, even if displaced. Articular fractures need to be reduced and fixed. Fractures in case of simple bone cysts are frequent. Dislocations are rare in pediatrics, and first episode must be treated medically, in case of new dislocation, surgical treatment should be considered. The notions of physiological laxity, pathological laxity, hyperlaxity and instability are fundamental for understanding pediatric shoulder pathologies.
