Origen anómalo de la arteria coronaria derecha del seno aórtico izquierdo

We report the case of a 32-year-old patient with ventricular fibrillation while running marathon. Diagnostic tests showed the existence of an anomalous origin of the right coronary artery from the left aortic sinus, with a non intramural course between aorta and pulmonary artery. Surgical repair was performed using a side-to-side anastomosis between the right coronary artery and the right aortic sinus. The patient recovered uneventfully. Anomalous aortic origin of coronary arteries is a rare congenital defect associated with increased risk of sudden death. Often myocardial infarction or ventricular arrhythmia are the first manifestations of the disease. Unroofing procedure is the commonest technique if the coronary has an intramural course. If there is not an intramural course and coronary reimplantation cannot be performed, this technique may be an acceptable therapy.
